Business phone
Owner's first & last name
Number of employees
Year founded
Billing address
Languages spoken
Credit card details
Professional Headshot for all Dentists and Associates
Calculate the Minimum Monthly ad budget for this campaign
The license number for each dentist at the practice
Select services they offer
Access to Google Business Profile
Access to their Google Ads Account (if applicable)
Copy of Certificate of Insurance / Professional License
List of accepted insurance providers
The email address for all patient interfacing doctors, associates, assistants & hygienists.
These email addresses are used to contact them to complete a background check required for the ads to run.
